The application forms on this site are available in Microsoft Word and Adobe Reader formats.
Microsoft WordIf you can open your application form in Microsoft Word format, you can edit it on your computer and email it back to Myrna.
If you do not have Microsoft Word (part of MS Works or MS Office) installed on your computer, we recommend that you download and install the correct version of OpenOffice for your computer. OpenOffice is free and completely compatible with Word documents.
Adobe ReaderAs an alternative, we also offer the application forms in Adobe Reader format. Adobe Reader is free and universal: it works on almost any computer. If you do not have a copy of Adobe Reader installed on your computer, you can download it here.
If you open your application form using Adobe Reader, however, you will not be able to edit it on your computer. You will need to print out the form, fill it out by hand, and mail it or fax it back to Myrna.
